The lives of over 30 people who perished in the Channel on November 24, 2021, could have been saved had a French warship responded to their Mayday call, an inquiry has revealed. This devastating incident, the deadliest ever Channel migrant boat sinking, highlights systemic failures and missed opportunities in the UK's search and rescue response.

The inquiry, led by Chairman Sir Ross Cranston, identified 27 men, women, and children whose bodies were recovered, while four others are missing and presumed dead. Only two of the 33 people on board survived, found in French waters nearly 12 hours after the initial distress calls.

Sir Ross's 454-page report concluded that the loss of life was 'avoidable' and 'some of the deaths could have been prevented.' He emphasized the need to end the practice of small boat crossings to prevent further loss of life.

The people smugglers responsible for the unsafe inflatable vessel and the inadequate safety equipment contributed to the tragedy. The boat became swamped and capsized during the journey from France to the UK in the early hours of November 24. The first distress call was made by a 16-year-old Iraqi Kurdish boy, Mubin Rizghar Hussein, at around 1:30 am, followed by a Mayday message broadcast by the HM Coastguard at 2:27 am.

The French warship Flamant, about 15 minutes away, did not respond to the distress call, known as 'incident Charlie.' The reasons for this inaction are under French criminal investigation, but the inquiry suggests that if the Flamant had attended, many more lives could have been saved.

Sir Ross also highlighted 'systematic failures, missed opportunities, and inadequate resourcing' in the UK's search and rescue response. The HM Coastguard was placed in an 'intolerable position' due to chronic staff shortages and overwhelmed staff at Dover, which directly contributed to the failure to rescue people in the water.

Despite these issues being known for some time, no action was taken, representing a significant, systemic failure on the part of the government, led by then-Prime Minister Boris Johnson. The Border Force rescued 367 people that night, while 757 people arrived in the UK after crossing by small boat.

The inquiry concluded that if a search for survivors had been adequately undertaken during November 24, including daylight hours, more lives could have been saved. While much has improved since the incident, Sir Ross has made 18 recommendations for improving search and rescue efforts for small boats.

This tragic incident raises important questions about the effectiveness of search and rescue operations and the treatment of refugees. It is crucial to address these issues to prevent similar tragedies from occurring in the future.
